As doenças da visão, que incluem, entre outras, a degenerescência macular da idade (DMI), retinopatia diabética, glaucoma, doenças hereditárias da retina, e doenças da córnea, afetam mais de 400 milhões de pessoas em todo o mundo. A maioria destas doenças não tem cura, os tratamentos disponíveis são escassos, e não são eficazes em muitos doentes. Além disso, os biomarcadores disponíveis para o seu diagnóstico precoce são diminutos, ou nem existem, havendo também necessidade de identificar novos biomarcadores para a progressão de doença e resposta à terapia. Deste modo, a clarificação dos mecanismos moleculares e celulares associados à fisiopatologia destas doenças, a identificação de novos biomarcadores, e o desenvolvimento de novas terapias avançadas são uma necessidade premente.
Muitos doentes sujeitos a cirurgia de catarata ou cirurgia refrativa sofrem de disfotopsia. Isto pode ser devido a mecanismos que ocorrem a nível neural, e a neuroadaptação e neuroplasticidade cerebrais podem representar um fator importante para que haja respostas favoráveis.
Além disso, uma vez que a retina e o cérebro partilham a mesma origem embrionária, e a retina, contrariamente ao cérebro, pode ser facilmente avaliada por técnicas não invasivas e não dispendiosas, a mesma pode ser explorada como uma janela para o cérebro ou um espelho do cérebro, para facilitar o diagnóstico de doenças que o afetam.
Os principais objetivos do Grupo são os seguintes:
- clarificar os mecanismos moleculares e celulares associados à fisiopatologia de doenças degenerativas da retina, dando uma atenção particular à neuroinflamação mediada por células da microglia, as células imunitárias residentes no sistema nervoso central, assim como à comunicação intercelular mediada por vesículas extracelulares, e por outros mediadores;
- Identificar novos alvos terapêuticos;
- Identificar novos biomarcadores de doença, em particular para as fases iniciais da mesma, mas também para a sua progressão, e resposta à terapia;
- Identificar e validar parâmetros clínicos apropriados para futuros ensaios clínicos na DMI;
- Dissecar a interação entre dieta, estilo de vida e risco genético na DMI;
- Identificar doentes em risco de demência, avaliando a retina de doentes diabéticos, e identificar vias fisiopatológicas comuns associadas à disfunção e degenerescência da retina e do cérebro;
- Desenvolver estratégias terapêuticas avançadas para queratites fúngicas e queratocone (soluções baseadas em nanotecnologia), glaucoma (promovendo a regeneração do nervo ótico), e retinopatia pigmentar (edição de genoma);
- Identificar novas estratégias que facilitem a neuroadaptação.
