PERSONAL STATEMENT


I have always considered since I was born that I had an obligation to pay back to Society and Mankind by contributing with new knowledge. I chose to do that in the field of Science and Healthcare.

After my medical licensure which was obtained in Coimbra, Portugal and during my residency in Ophthalmology I applied to join the Research Unit of Norman Ashton in the Institute of Ophthalmology and Moorfields Eye Hospital with Barrie Jones, both in London, Great Britain.

I remained in London from March 1963 until July 1966. There, I developed, under their guidance, my enthusiasm for the study of the retina and focused on diabetic retinopathy as my primary area of interest. There, I received my Ph.D degree.

I returned to Portugal to finish my Ophthalmology residency. In Coimbra, I also had a Dr. Medical Science examination at the University of Coimbra. My research activities were then interrupted for a period of over 2 years because I had to spend a 2-year period of mandatory military service in Luanda, Angola, Portugal. After returning to Portugal I was named Director of the Department of Ophthalmology in the University Hospital of Coimbra. Between 1978 and 1986 I had two separate periods of two years as full Professor of Ophthalmology at the University of Illinois, in Chicago, USA. One between 1978-1980 and the other between 1984-1986. There, I had the opportunity as Director of the Retina Service and of the Deicke Eye Centre to work and collaborate with top Ophthalmologists and Researchers such as Morton Goldberg, Mark Tso, Gholam Peyman, Lee Jampol, etc. It was a unique experience and a unique opportunity to discuss ideas and perceive new directions for new contributions. These periods offered the right environment to further develop my interest for retina research.

Upon my final return to Portugal in 1986 I had the opportunity to establish in Coimbra a research environment that I tried to copy from the USA. As Portugal entered the European Union there were opportunities for funding Biomedical Research and I was able to organize research groups and prepare applications that were approved and led to the funding of two research Institutes, AIBILI and IBILI (Institute of Biomedical Research on Light and Image), the first dedicated to applied research and innovation and the second to laboratory research, bringing together expertise in Ophthalmology, Biomedical Engineering and Pharmacology.

My personal interest for investigator driven clinical research led to the organization of EVICR.net, a network of over 90 clinical trial research centers in Ophthalmology. This organization was crucial to support clinical research trials that were performed through multinational European Union Research Funded in Projects such as Ocular Fluorometry, EuroEye, Eurocondor, Macustar and Recognised.

I can say now that I have been able to reproduce in Europe the Research Environment that I was exposed to in the USA thus creating the conditions for new contributions to Science.

CONTRIBUTIONS TO SCIENCE (up to five)


When, in England, working under the supervision of Norman Ashton I was challenged to focus on the retina and more particularly in Diabetic Retinopathy. In 1965 I published my first work identifying the blood-retinal barrier. We showed that this critically important barrier was located in the endothelial lining of the retinal vasculature and in the retinal pigment epithelium. In ultrastructural studies we demonstrated, for the first time, that the morphological basis of the barrier was the presence of specialized tight junctions between the retinal endothelial cells (Arch Ophthalmol (1965), 73:211-223; Br J Ophthalmol (1966), 50:411-453; Exp Eye Res (1966), 5:229-234; J Path Bacteriol (1967) 93:645-652). This observation was later confirmed to be present also in the brain and helped define the blood-brain barrier function. In physiologic studies, performed with David Maurice, we were able to show for the first time that transport of fluorescein dye and organic ions was an active process and that these organic anions were pumped out of the retina into the systemic blood stream against substantial concentration gradients, thereby protecting the retina from chemical damage. This research led, later on, to the development of vitreous fluorometry and its use to follow diabetic retinal disease (J Path Bacteriol (1967), 93:645-652).

I introduced vitreous fluorometry to examine and quantify alterations of the blood-retinal barrier in a variety of retinal disease with special emphasis on diabetic retinopathy (Br J Ophthalmol (1975), 59:649-656; Ophthalmology (1979), 86:264-275; Arch Ophthalmol (1983), 101:1753-1756; Arch Ophthalmol (1983), 101:1757-1761). The instrumentation prototype was initially put together in Coimbra, Portugal but it was possible to develop a commercial vitreous fluorometer, the Fluorotron, during my permanency in Chicago, USA, with the collaboration of a U.S. company, Coherent. Afterwards, but continuing this work, I was able to identify and characterize the role of the blood-retinal barrier in the development of cystoid macular edema (Surv Ophthalmol (1984), 28:485-492).

In 2002, another contribution that followed the work on clinical measurement of changes in the blood-retinal barrier, was the development of multimodal macular mapping, bringing together for the first time (Surv Ophthalmol (2002), 47:580-589) different imaging modalities to combine information on retinal disease such as diabetic retinopathy and age-related macular degeneration. Using this new methodology, in 2004, our group was able to propose different phenotypes of nonproliferative diabetic retinopathy associated with the development of macular edema (Arch Ophthalmol (2004), 122: 211-217). Following on this line of research, in 2009, we developed an original method to measure microaneurysm turnover in the initial stages of DR, being able to predict risk for development of clinically significant macular edema. The Retmarker was patented in the USA and Europe (Ophthalmol (2009), 223:284-291; Ophthalmol (2009), 223-:292-297; Diabetes Care (2013), 36:1254-1259). In 2013, we were able to identify and characterize three major phenotypes of diabetic retinopathy progression which showed different risks for development of sight-threatening complications. This work opened the way for personalized management of diabetic retinopathy (IOVS (2013), 54(7):4595-4604). More recent publications have confirmed these findings.

In 2016, our research group developed a new automated method to measure noninvasively abnormal extracellular fluid in the retina, OCT-Leakage. This method based on OCT-images has been patented in the USA and Europe (IOVS (2016), 57:6776–83; Ophthalmology Retina (2017), 1:395-403; Ophthalmology Retina (2017), 1:395-403).

The characterization of progression and regression of nonproliferative diabetic retinopathy has been the focus of recent research work.
